La Purisima Catholic School Application Checklist
Thank you for your interest in La Purisima Catholic School. To start an application, please:
_____ Complete and return the New Student Information Sheet and Preschool Application (if applicable) to the school office with a $375 non-refundable Registration fee (per child). You must obtain the Application from the school office.
Please submit the following paperwork to the school office. All paperwork must be submitted together (except for the teacher evaluation).
Please note: incomplete applications will not be accepted.
_____ Copy of birth certificate
_____ Copy of baptismal certificate
_____ Copy of First Communion certificate
_____ Copy of current, up-to-date immunizations records
_____ Copies of 2 complete years of standardized test scores
_____ Copies of 2 complete years of report cards
_____ Teacher Evaluation
